We are seeking aPRN Physical Therapist (PT)to join our dedicated rehabilitation team. This position offers flexibility and the opportunity to make a meaningful impact on residents' lives in a rewarding setting.
Position Summary:
As a PRN Physical Therapist, you will evaluate and treat residents to improve their mobility, strength, and functional independence. You will work collaboratively with a multidisciplinary team to deliver high-quality, individualized care.
Key Responsibilities:
- Perform comprehensive evaluations of residents’ physical abilities and needs.
- Develop and implement personalized treatment plans to improve strength, mobility, balance, and overall function.
- Provide therapeutic interventions, including exercises, manual therapy, and training on assistive devices.
- Monitor residents’ progress and modify therapy plans as needed to achieve goals.
- Collaborate with an interdisciplinary team to ensure holistic care.
- Educate residents and their families on therapy goals, exercises, and strategies for continued improvement.
- Accurately document evaluations, treatment sessions, progress notes, and discharge plans in compliance with regulatory and facility standards.
- Participate in quality improvement initiatives and contribute to the rehabilitation department's success.
Qualifications:
- Doctorate or Bachelor’s/Master’s degree in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in the state of Colorado.
- Experience in skilled nursing or rehabilitation settings is preferred.
- Strong communication and teamwork skills with a resident-centered approach.
- Ability to work independently and adapt to a flexible PRN schedule.
- Dedication to providing compassionate and effective therapy services.
