Case Management Department – Unit Notes
Preferred Experience & Skills
- Minimum 1 yearofinpatient case managementexperience required
- Prior critical care experiencepreferred
- Knowledge of:
- CarePort
- MCG
- InterQual(strongly preferred)
- Prefersome UR experience
- Strongcritical thinking,time management, and apositive attitudeare essential
Required Certifications:
- BLS(Basic Life Support) – Required
- PRI & SCREEN Certifications– Strongly preferred
- Can be completed online through the School of Albany
- Typically takes a few hours each; approximately $300 per certification

Role & Responsibilities
- Focused ondischarge planning; not responsible for utilization review (UR)
- Work collaboratively withinterdisciplinary teams, including physicians and social workers
- Expected toround on the unitwith the care team
- Make referrals and assist with discharge tohome, community settings, or SNFs
- May help with:
- Housing placement and Shelter Packets(for patients experiencing homelessness)
- Hemodialysis packets
- KCI wound vac referrals
- A referencebinderis available to support daily duties
Caseload & Patient Population
- Typically coversMedical-Surgical (Med-Surg)orTelemetry units
- Patient load varies by unit setup:
- Standard Floor: 1 CM & 1 SW |Caseload: 17–25 patients
- Pilot Floor: 1 CM & 2 SWs |Caseload: 12–17 patients
- Must be able to independently manage acaseload of at least 20 patients
- Patient population includes those underInternal Medicine and Family Medicine

- EMR System:Allscripts
- UR Software (Preferred):CarePort, MCG, InterQual
Department & Management
- Reports to:Manager Reiny
- Work Location:Inpatient units only
- Utilization Review (UR):Handled by a separate department
Schedule & Shift Info
- Shift Hours:Monday–Friday, 8:00 AM – 4:30 PM
- (7.5-hour shifts with a 1-hour unpaid lunch break)
- Weekend Coverage:Rare, with manager approval
- Weekend shifts may involvecovering multiple units
- Holiday Coverage:Rare, with manager approval
Dress Code
- Attire:Business casual or scrubs (except Royal Blue or Maroon)
- Neutral-colored scrubs preferred
- Lab Coat:Provided by the facility
Orientation & Onboarding
- Hospital Orientation:2 days (includes EMR training)
- Unit-Specific Training:1–2 weeks with a preceptor
- Duration depends on individual progress
