📝 Job Description:
The Registered Nurse will provide skilled nursing care to clients of all ages in their homes. This role is responsible for the assessment, planning, coordination, and delivery of care in collaboration with the interdisciplinary healthcare team. The RN ensures the highest quality of care while maintaining patient comfort and dignity.
Essential Functions:
- Patient Assessment: Evaluate home health and/or hospice patients in both stable and emergent situations to determine physical, psychosocial, and environmental needs.
- Plan of Care (POC): Develop, implement, document, and evaluate individualized care plans to maintain quality and continuity of care.
- Data Collection: Gather comprehensive data including physical, psychological, social, spiritual, and cultural factors, and incorporate into the POC.
- Documentation: Maintain accurate and up-to-date clinical records, including visits, patient interactions, goals, and any changes in condition.
- Infection Control: Provide care using infection prevention practices in compliance with OSHA standards to ensure safety for patients and staff.
