Work Experience:
Demonstrated clinical autonomy in medication therapy management and outpatient care
Education:
PharmD (or B.S. Pharm prior to 2000) from an accredited institution
License:
Active Wisconsin Pharmacist license
Certifications:
BLS required; board certification (e.g., BCACP) encouraged.
